also they asked for total yearly gloss salary. if i just put the rate of pay and hrs per wk will that meet the requirement ? or do i hv to state the total amount ?

If you're talking about a form, put your current rate. If you're talking about the letter of employment, you can put both. ("From DATE1 to DATE2, Kittens was paid WAGE1 per hour. Since DATE3 Ktitens has been earning WAGE2 per hour."

It doesn't hurt to state an estimated annual earnings (but that is not a salary).
 
Exactly how jes_ON mentioned. On my reference letters they both mentioned my hourly rate and what I made with the company.
