Visitor Visa Passport request - USA - What to submit?

Hi, I applied online from the US for my wife and myself. Could you confirm the items I need to include? Also can I include both our documents/passports in the same package?

I have the following:
1. Passport request letter for me and my wife
2. Our Passports
3. Prepaid return envelope

Also, I plan to send to the LA office:
Address:
EAPP
Consulate General of Canada
Visa Section
550 South Hope Street, 9th Floor
Los Angeles, CA 90071

Please confirm if the above information is correct and if there is any additional information I am missing.

Hello, i don;'t know if that address is correct as i went to LA Embassy today to submit my passport and they told me to do that in El Segundo office which do this stamping for approval application.
Kindly check again, i would recommend.
