Pretty sure they are asking for a new medical. They have the previous one on file. You don't have to send your passport to India. The extension would be processed in Canada.

You can do your medical in India. Depending on how long you are spending in visa you may need to apply for a new visitor visa and not an extension. You could do a CSE and ask the processing to be done in India. Changing addresses tends to confuse the system though.

If you leave Canada while under implied status or an extension, you no longer have status in Canada. Returning to Canada would be considered a new entry and you would be assessed by CBSA as such. You would require either a valid TRV or a new TRV to return. An extension wouldn’t grant you entry back into Canada. Depending on how soon you plan to leave (and if you plan to return soon after) you may not need a medical.
