Visitor visa confusion

I'm confused about this, mainly the additional family information form. (5406).

I'm divorced and I don't know how to fill in this form. If i don't fill anything at spouse, it sends me to a note where I must certify that i don't have a spouse or ex-spouse. But i do have an ex-spouse (he's not dead :)). Do I have to fill him in the spouse section and mention we're divorced?

Please help.

This one cracks me up, I can't believe CIC still hasn't fixed this!! Perhaps it's a reading test...

I wrote "see attached" in both places, and wrote an explanation that I couldn't fill in the spouse because I don't have a spouse, but that I couldn't sign the statement either because I have an ex-spouse. I then gave the info for my ex-spouse, and the date we were divorced, and signed my self-prepared statement.
