Visitor visa based on work permit

hi, i am an immigrant student, now i am a work permit holder, i need to apply for visa extension, on CIC website while applying for that i am getting requested to fill form named as "IMM5257E" is it correct form to fill?

if it is correct ?

in that form in details to visit Canada, it asking to provide name and address of person or institution which i will be visiting, since i am just a worker what do i put here? i am not normal visitor!

any answer is greatly appreciated. thanks.

Yes it is the correct form. I faced the same situation. I submitted a wrong form and had to withdraw the application. And the place of your visit, I put down my work name and address. It should be fine.

So I called CIC and also sent them a CSE to withdraw my application. After waiting for an hour, they finally received the call and the lady said that the application will be withdrawn shortly and I can apply with a new one. My refund will be sent back to me in 16 weeks!
