Visa validity prior to first landing?

I have heard that it is one year from the date of your medicals?

Is this the day your medicals were assessed by the visa officer, or the date the information sheet was issued?

The distinction is important, as my wife received her upfront medical in June, and I need to know if that means(assuming the visa is approved), she will need to land in Canada by June of 2016?

The visa validity is determined by either one of the two, which ever comes first, one year from the medical date or the expiration date of the passport.
In your case your wife's visa will be valid till June 2016 except her passport expiration date is earlier, in which case the expiration date of her passport will determine the visa validity.
In exceptional cases, the VO might decide to issue a visa even though the medical has expired.
