My visa got refused and I received my passport today. There is also a refusal letter. On its second page, there are empty boxes with sentences like travel history, study duration, medical examination etc. There is also a cross on one box.

What was the reason for my refusal? Is it the sentences next to empty boxes or the cross ones?
