My mother in law is in Canada for almost a year. 6 months ago we applied for extension and it was approved. Now we want to extend again for another 6 months. On the document list there are asking for a medical report if she did the medical exam in the past 12 months. She did it when we extended her stay. At the end of her medical I think the doctor signed the form. but that form I can not find it anymore. what should I do to prove that she made her medical exam on this new application?
