Using previous medical exam results - documents required

Hi everyone,

I'm preparing documents for EE FSW and I have a question about proof of previous medical exam.

So the official website says if I'm currently living in Canada and have done a medical exam within 5 years, I might be able to use the previous results for my EE application.

It says here that:
When the panel physician completed your exam they would have given you an
  • information printout sheet and
  • IMM 1017B Upfront Medical Report form
However, I only have the first one, and was not given the second document (IMM 1017B). Does any one have similar experience or suggestions?
