Hi,
While filling out the application form 5257E the "CONTACT INFORMATION" section tells:
"If you wish to authorize the release of information from your application to a representative, indicate their e-mail and mailing addresses in this section"
So I'm not sure does it apply to the entire contact info section or just first question. I filled out the "Current mailing address" using representatives address but I don't know what should go in the "Residential address".
Could anybody help with this?
