use of a represenatative, help!!!

I have a question regarding the form IMM5713E (use of family member representative form).
I'm representing both my mother and brother for a visitor visa.
I'm assigned as a representative for my mom using IMM5476E. Who should be assigned in IMM5713E for my brother? Me or my mom?
