Hi,

I am US trained resident physician in Neurology with a total of 4 years of specialty training and 1 year in sub-specialty training. I am a foreign medical graduate originally from India. I am married and looking forward to migrate to Canada and practice as a neurologist. I would appreciate if someone can share the process and additional formalities that are required to migrate and start practicing neurology in Canada for someone who is in my shoes.

Your best option is to look into applying through the Express Entry immigration program:

You should also investigate what it will take for you to work as a doctor in Canada. You'll need to take several steps to be certified here.
 
You should be looking to do a fellowship. Fellowships are basically expected in most specialties in Canada these days.
 
You should also see if Canada is hiring neurologists. There are too many graduates for positions in some specialties.
