US ID proof for commuters for doctor's appointment in the US

Hi All,

Those who have health insurance through US employer but live in Windsor and commute to Michigan, what do you show as ID proof for Doctor's appointment in the US?
They seem to be accepting only a photo ID with a US address on it.
