Please help... I have submitted my PR application last week after receiving ITA. Today I got below message to upload 2 documents:

Doc no.2 IMM5475 is fine, I have downloaded from CIC website, will fill and upload
Doc no.1 They just put my Unique Client Identifier Number and My name: Which document is this? Where do I find it?
