Urgent response appreciated-Filling form after getting ITA

Hi,

I have received the ITA in September 2017 and now need some guidance on filling the information. I have the following queries:

• I had made an express entry profile last year which got expired. Now i have created a new fresh profile and have received the ITA as well. Should i write Yes or No on the question "Have you applied for immigration before?" In my opinion i should write "No" as i did not receive the ITA before.Please advise.
• In the address history they have asked for 10 years history. What if i am living in the same house for 30 years, what should i mention as the start date? 10 Years or 30 Years.
• In the education history i there a requirement to mention about matriculation? Though, my ECA was not done for matriculation. Or should i start education history from Intermediate? I had not included Matriculation on the express entry profile.
• I am a doctor by profession. Got my degree in Aug 2005 and started working in Aug 2005 as well, however, the license certificate was issued in Sept 2005. What should i write as a start date when i became qualified to practice?
• There is a question on research after work history. What should i write here?
• Personal History- Should i mention about my vacation travels here? What else should be mentioned here?
• Travel history- If i am resident of another country and make frequent trips to my home country. Do i need to mention it in the travel history?
