Urgent:Regarding Post ITA Legal Name change doc

Need help on post ITA legal name change document?

I'm using my maiden name on all my documents including education, passport, wp, employment records etc. Also based on maiden name i got PNP. Now after filling the post ITA questions, it's asking me to attach Legal name change document. I haven't used after marriage name anywhere on my documents except that I have used after marriage name only on my minor kid birth certificate and passport document.

What I need to provide now for legal name change document since I haven't changed my name on my docs anywhere. Will an LOE will help stating that I have used after marriage name only for my kid documents and not anywhere?

Please suggest on this urgently.

You need a notarized affidavit. Also, write a LOE.

It's called name change affidavit. It's very easy to make one with the help of a notary. You can get it done within an hour.

Does Name change affidavit means that person is going to use the changed name from here onwards? Since I haven't used my after marriage name anywhere on my documents except it was used in kid's document.

Also, can you suggest what format it need to be to justify the scenario above?

If this is the case then it will entirely make my case more worse. As I'm the primary applicant I don't want to complicate things much. Moreover I have specified my after marriage name in the IMM* docs used for wp, worker visa extensions where it states - "Give any other names used(maiden,nicnname,alias etc)?. My UCI can pull up the other name, if CIC need to be verify that way.

Not sure what exactly i have to here Please guide?

Oh if you are a female and changed your name after marriage, then marriage certificate would be enough as a proof of document. You can write an LOE with that.

For safe side, you can submit marriage certificate, notarized affidavit and LOE. Take help of the notary, explain him the situation and he should be able to make an affidavit explaining your situation.

I am not aware of that. Check with the Notary. They will know it. You can find Notaries on Kijiji.
