URGENT: medical upfront

Hello all,
I had an issue with medical check up form, the doctor checked the immigration type in form as Visitor, while I am a spouse of an applicant so I think it should be Family? He said he will correct it on the same form Do CIC accept changes to a form like writing over.

Is the primary applicant through Express Entry? Then this should be worker category or Express Entry category

It's for express entry, my husband is the applicat so he checked worker and I should check family? Or worker too??

The details on IPS should be for - # IME: Non EDE & # Upfront Medical type: Worker for both.
