Urgent Help Required regarding form 5440

I am sponsoring my daughter who is 6 months Old. Do I need to fill form 5440 (Sponsor Questionaire).

Guide says that if you are sponsoring dependent children It is required. However, heading of form doesnt say Dependent Children.

I asked 4 CIC agents and 3 of them said not required, but one said YES since it is mentioned in the guide.

I am completely confused. Please help. Also if I need to fill it then:

Q17) Have you been in married or common-law relationship before?
Does this question apply to present spouse or previous spouse? I am married only once and still married. So my answer should be YES or NO?

im not sure about form you should submit or not but answer to question 17 is no.
they are asking about previous relationship.
