URGENT HELP !! ADDITIONAL FAMILY INFORMATION NATIVE LANGUAGE

Hi,
I need to fill the (IMM5406) form . I recieved the request from CIC a day ago.
I am an Indian . My native language is hindi but my mother tongue is Urdu . I am fluent in both the languages. Which one should I be filling my application in ? Also, I unlike other applicants, I have not received the schedule A Form Request . Should I be expecting it in the near future ? Since I only have six days more to submit the (IMM5406) form , Quick and prompt reply will be highly appreciated.

What language did you mention in your IELTS and while filling your eAPR? It's best to be consistent in all your documents.
