Urgent advise on wife's name change

Hi all,

I'm going to submit the application by Monday and I need your advise urgently. There are tons of posts regarding gift affidavit but I couldn't find any information regarding the name change affidavit. The only documents we have is our marriage certificate (which also shows her maiden name) and her old expired driver's license she used before we got married. She got her passport after we got married so we have no other legal document showing her maiden name. We're living in the U.S., not in our home country. How do we get the name change notarized here? Is there an affidavit template for it? Can you please help? I don't have much time left as I have to submit our application by Monday. Thank you very much

Why do you think you need a name change affidavit? The marriage certificate showing her maiden name and new married name is sufficient proof.

MC its enough, only maybe her Birth certificate or old documents with maiden surname?!

A few people strongly recommended to get an affidavit and support it with a birth certificate. But we don't have her birth certificate with us. Here in the US, notaries don't give any advise about the affidavit format either (or at least the ones I went to). I found a few templates on the internet but they are all in the format that Indian government requires. They also require to put a joint photograph with the spouse. I'm not sure what a joint photo means either. I guess it is very common in India. In our country, marriage certificate is enough to prove the name change but I'm not sure what CIC requires.
I'll definitely attach our MC as a proof but I want to get an affidavit to be on the safe side.

You don't need an affidavit. Again, the marriage certificate is sufficient proof of the name change in Canada.
