Urgent: ADR Requested

Hello Seniors,

I got the following msg in my CIC account today:

Dear XXXXXXX,
This is in reference to your application for permanent residence in Canada. In order for us to continue the review of your application, we require the following documents:
 MY UCI Number - My Name:
 Release of Information IMM5475: A signed and completed Authority to Release Personal Information to a Designated Individual form (IMM5475) bearing original signatures.

This form has field asking for :
1. Name of Office where application is submitted
2. Location of Office
3. Type of Application

My problem is that I do not know which VO is processing my case because I submitted everything online (duh!).

Please help. They have me given me 3 days to send this document.

At the very end of the request letter there probably is mentioned the location of the office. If you're not certain say unknown and attach a letter of explanation saying what you said in your post.

I checked the letter again. Only their website is mentioned there and nothing else.

Send an inquiry via webform. Mention urgent and that your are requested this document which needs the location. If you don't get a reply within time submit anyway with what I mentioned.

Or you can send it right away explaining that you don't know.

Weird. Why would CIC ask for your UCI number when they have it on file? What is the email of the sender?

This was in my CIC account...My UCI and name was mentioned there..i just replaced it..

Ok, sorry. In that case, if you don’t know your VO, you can just leave it blank. Good luck.
