Hello, I am planning to get upfront medical exams for my parents at Apollo Ahmedabad for their Super Visa application. According to the IRCC instructions, "When you have an upfront medical exam, the physician will ask you to fill out an IMM 1017B Upfront Medical Report. You must include a copy of the report with your paper application. If you apply online, you must upload the report before you can submit your application."
My question is, does the hospital provide the IMM 1017B report form to the applicant?
If they do!!! I am planning to upload it in the "Other Documents" section under their Super Visa application. Should I also raise a web form as soon as I submit the application? Please advise.
