Upfront medical confusion

Hello friends
I am july 2014 applicant. I did my medical in march 2015. Later i added my spouse to my application, but medical for my spouse is not been requested yet. So I decided to do an upfront medical for my spouse. My spouse lives in India.
I have couple of questions
1: what should I mention in "class" if doctor asked for it in india?
2: what documents do i need to send to CIC after completing her medical examination to let CIC know about this?
3: where to send these documents? Any email address or physical address??
