Hey there!

Hope you are all well! :)

I'm considering to have my medical check within the next two weeks before my sponsor and I get married. Do I have to wait till I get my new passport, as my family name will change after the marriage or can I take my current one with my birth name?
We're going to apply from outside Canada and I live in Germany...
How long does it take to get the results? Or does the physician send the results straight to the visa office in Vienna where my application will be processed?

You don't have to change your name after getting married. To make things easier, you may want to go through changing your name after you have PR.

The medical exam results are sent to the visa office, so you don't have to worry about that. They will give you a receipt after you do the medical, which will let the vo know u have completed the medical exam, and also allow them to access the results.

The doctor who performed my medical sent the results for my e-mail on the next day just because I asked him to do it.
If you have some health problem, you'll be contact for sure.
