Hi,

CIC just send me an email asking the following:

This is in reference to your application for permanent residence in Canada. In order for us to continue the processing of your application, we require the following documents:

Ø ######## - Spouse Name:
Ø Client Information: Updated residence history for past two (2) years. You can send a scanned copy as a reply to this email.
Ø


Can any one direct me as to if this is a specific form that they need or can I make a generic one(in excel) and can send it to them.

Please advice.

They need actual form. Which I think would be Background Declaration from.

I received a similar request today at 12:40 PM EST. I have sent the information in a word document.

Do let me know what info you send. Was it simply address for your spouse for the past 2 years. Did you also include the work information if your spouse was working after the application ?

You can send information like the following, I was asked and provided the same....
DL, Bank Account statement with address, Home insurance copy, Rental / lease agreement copy, Marriage Certificate, Passport Port of entry/exit stamps, Work permit or Visitor record copy and any other canada documents which has address and spouse name.

CIC already has the marriage cert and passport copies( with PR application and for sponsoring spouse) . Did you send a word doc with dates(from and To) for address ?
They asked for an updated residence history- can i make a simple word doc with the dates for where the spouse was in the last 2 years? Does it need to be signed ?

Hi there,

I have sent the 2 year residence history in a word document in the the below specified format

Name – XXX XXX
Reference (Application) Number - XXXX
UCI – XXX

2 Years Residential Address History (Tabular)
Address From To


********************************************

In my opinion, we are not required to send Schedule A – Background/Declaration [IMM 5669] or the scanned copy of the address proof as it is not asked for.

Experts pls advise.

There is no need of signature however there is no harm in providing it.

I got the same request on oct 06,2014to provide spouse residence history for last 2 years. I have sent the details on ooct 20,2014.

Did any one of you got any updates from CIC.
I haven't got any updates.

I don't know, how much time CIC will take to process the application. Already medicals are done RPRF is paid.

I got a request to provide the residence history for the past 2 years for self and family on the 08-Oct-14. No updates after that.

To the best of my understanding, they had asked it because our medicals our expiring in Nov-2014.

Questions
Have you applied under CEC and when did you apply?
Is you and your spouse medical set to passed in GCMS?
When did your spouse got her medicals done or when is it expiring per GCMS Notes?
