I requested GCMS notes a couple of weeks back, and just received the following email:

The consent of <MY WIFE> is required to process your request and allow us to release your spouse’s information to you. To be valid, a consent must be signed, recently dated, contain the printed name of the person giving consent and identifies you as the designated individual. Please ensure that the consent form is signed by all applicable parties, and has the signature date and date of birth.

No problem, but they don't mention to what address (physical or email) this needs to be sent to though. Do I need to send a physical copy or can a photo/electronic version be uploaded? Would a fax be ok? :D
