My wife did her upfront medical in Armenia and on her certificate it has a "UMI" number.
On the certificate it also states "If you have not yet lodged a visa application, you must provide the health identifier on the top of this letter (i.e. UMI, UCI, IME) to CIC. Please make sure that you attach this letter to your visa application before submitting the form to CIC. This will facilitate the processing of your application".
Now, do I need to provide this number anywhere on her forms? It's obviously not the same as the UCI number, as that's XXXX-XXXX format (unless it's changed?). Or do I simply attach this certificate (which I'll be doing anyway) to my application and not bother with anything else.
