Would my wife have a uci number? She became a citizen in 1996 we have her card and letter but can not find anything with xxxx-xxxx on it. Is it alright to leave it blank?

You may leave this blank - but in addition to this you can attach an explanation letter along with app and this will help CIC to ensure that why this info is missing on form.
