TRV + medical , please help

I did medical of my wife from a CIC authorize panel, I was planning to apply for her OWP but now I decided to apply for TRV not OWP

While I try to open a profile for my wife online, they ask me if medical was done in last 12 months

What answer shall I chose for TRV

1)If I chose YES, will they think why medical was done? May be VO will think that she has other intention?
2)If I chose NO, will it indicate misrepresentation of information?

Please help

You could explain the situation, in a separate document along with your wife's TRV application.... and how the circumstances have changed since then.

Its always best to be honest and truthful
