TRV from within Canada

Hello All,

I applied for my PR and waiting a decision and also applied for BWP which expires 2018.I had a TRV but it is expired now, is the correct form to fill out
Application to Change Conditions, Extend my Stay or Remain in Canada as a Visitor or a Temporary Resident Permit Holder (IMM5708)

Also which selection should I choose?
Extension of my temporary resident status
Restoration of temporary resident status as a visitor
Initial temporary resident permit or extension of temporary resident permit
