Traveling back to US after getting PR through Refugee or Humanitarian that crossed from the US

Has any former refugee claimant (who entered Canada from US) or H& C permanent resident successfully obtained a US visa and visited the US? Did they need to get a waiver? Whats the process? Thanks in advance.
