Travel History and Lost Passport

Hi Team,

I have received ITA and I am in the middle of my application submission.

I do have a travel history where my passport was lost. How to handle this situation in my application?

Q1. Should I mention it in my travel history and mention the case in LOE?
Q2. Do I need to provide a Police report or FIR copy for the same?

Please help!
