I got my transcripts from relevant universities. but i have not filled any academic report form, as available on WES site. Is the form necessary to be attached with the documents??

It is not necessay to fill form. You just send your transcripts and legible copy of Degrees with reference number mentioned on it. If you have doubt that date of birth is not written on transcripts do send your passport copy or 10th certificate.
