Transcript Form for Credit based MA course in Mumbai University??

Hello,

I did my masters in Hindi from MU.
Now in the last two decades, nobody has gone out of India who belonged to MU Hindi Dept.
And I have to now send my transcripts to WES Canada, so I am in a fix, because NOBODY in Hindi dept has any idea of the transcript format (office assistant to HOD, all clueless. They keep referring to transcripts as TC and then they gave me a form to fill which they suggested they could stamp and sign and seal for me. I am pretty sure this will get rejected!!

And I went to multiple other departments, including Exam Control building to ask about the format so I could give the format to my dept to make the transcript, but no body is helping.

Could someone PLEASE PLEASE PLEASE help me with the format? I hope some one has an extra copy of the transcript you sent which I can use to get mine made? I just need the format.
I AM LOOKING FOR FORMAT FOR A CREDIT BASED MA COURSE. which is NOT First Class or Second Class based but Grade and GPA based).

There is no such transcript format. Its basically your Detailed Mark Sheets and Degrees stamped and signed by Registrar.
