I am looking to bring my mother to Canada on Super Visa. I am a Canadian citizen, my wife is PR, our daughter a Canadian Citizen.
It would be of great help if someone can answer me regarding the following :-

1) Can the application be filed in Canada by getting all the documents couriered here as my mother cannot do it all by her selves especially with confidential information?
2) If the application cannot be filed in Canada then the details of documents i need to send there (like invitation letter , medical coverage for one year salary proof etc.) whether to be notarized or not?
3) Info on medical coverage offered to foreigners by Canadian companies.
4) Family Information IMM 5645: My mom has sisters and brothers who live in different places and few elder's DOB is not even recorded. So is it necessary to fillout the document with their details? If yes, then is there any other alternative for DOB's.
5) What is Schedule 1 - IMM 5257?

Dear Mr. Naveen,

I have read your post. I am also trying to get some information on supervisa. I am a Canadian citizen and my husband was holding a PR card which expired in June 2008. Now he wants to surrender the PR card and apply for supervisa. My son is working in Canada and he can sponsor him. The point about the insurance is not
clear. Can we take an insurance from the country of our residence or does it have to be from Canada?

your husband needs a PR travel document to return to Canada. Go to your nearest embassy to apply.

If he goes and applies for TRAVEL DOCUMENT then will they question him as to why he did not enter Canada after dec 2003.

also will they question him about his tax filing? he filed for taxes till 2005 and then he stopped.

I also stopped after 2008. But I have not declared myself as NON RESIDENT.

will applying for travel document lead to any problems? or should we cancel PR by signing the voluntary declaration of failure to satisfy residency obligation.

what is the best thing for him to do?

Hi Dreddy.. Let me answer your question one by one. My mom's visa was approved this morning. i decided to get her the single entry first as there was no medical exam or interview. She will be coming this April. Next year I am planning to let her apply for multiple visa..

1. You cannot file the application from Canada. The application should come from the country of origin. My suggestion is fill up the form electronically here in Canada, (some has bar code on it) and let your mom just signed it..

2) My letter of invitation was notarized. We paid $20. T4 if you have, salary proof are also needed. With the super visa, the minimum insurance is $100,000.

4) You need to fill up IMM 5645. My mom has 8 siblings and I put all their DOB

5) IMM 5257 is the application form for tourist visa. It is usually 6 months but you can extend for a year.

In short, if your mom will stay for a maximum of 2 years here in Canada, let her apply for super visa. But remember that the medical coverage is expensive because it should be minimum $100,000. If you would only like her to visit you for a few months, I will just go with the regular multiple visa. I got my mother $25,000 medical insurance with 0 deductible. It costs me less compared to super visa.
