Super Visa

I am looking to bring my mother to Canada on Super Visa. I am a Canadian citizen, my wife is PR, our daughter a Canadian Citizen.
It would be of great help if someone can answer me regarding the following :-

1) Can the application be filed in Canada by getting all the documents couriered here as my mother cannot do it all by her selves especially with confidential information?
2) If the application cannot be filed in Canada then the details of documents i need to send there (like invitation letter , medical coverage for one year salary proof etc.) whether to be notarized or not?
3) Info on medical coverage offered to foreigners by Canadian companies.
4) Family Information IMM 5645: My mom has sisters and brothers who live in different places and few elder's DOB is not even recorded. So is it necessary to fillout the document with their details? If yes, then is there any other alternative for DOB's.
5) What is Schedule 1 - IMM 5257?

If ur from India

you can ask ur rellative/friend to submit the application
even travel agency will do that

u need to submit app. at vfs center.

visit www.vfs-canada.co.in

in that check download(on left side) click that and check Document check list for PARENTS & Grand Parents
u need to submit following forms

Visitor Form
Family Information Form
Schedule 1
along with the documnts mentioned in the check list

you should also submit LETTER OF INVITATION...Plz check the following site to find out what should b included in the letter: http://www.cic.gc.ca/english/visit/letter.asp

you should meet MINIMUM INCOME THRESHOLD to invite your parent.

The medical coverage required is listed on the CIC site. It is also available on my website at http://www.travelinsure.ca. Basically, a minimum of $100,000 of coverage must be in place for one year, and proof of that must accompany the application.
