I am looking to bring my mother to Canada on Super Visa. I am a Canadian citizen, my wife is PR, our daughter a Canadian Citizen.
It would be of great help if someone can answer me regarding the following :-

1) Can the application be filed in Canada by getting all the documents couriered here as my mother cannot do it all by her selves especially with confidential information?
2) If the application cannot be filed in Canada then the details of documents i need to send there (like invitation letter , medical coverage for one year salary proof etc.) whether to be notarized or not?
3) Info on medical coverage offered to foreigners by Canadian companies.
4) Family Information IMM 5645: My mom has sisters and brothers who live in different places and few elder's DOB is not even recorded. So is it necessary to fillout the document with their details? If yes, then is there any other alternative for DOB's.
5) What is Schedule 1 - IMM 5257?

2. She has to submit it to Canadian Mission in her country of residence.
3. Attach all documents needed
4. Show at least the year of birth and moth if known
5.Schedule 1 – Application for a Temporary Resident Visa Made Outside of Canada (IMM5257 – Schedule 1)
Who must fill out this application form?

This form must be completed by:

the principal applicant;
spouse or common-law partner (whether accompanying you to Canada or not), and
dependent children aged 18 or over (whether accompanying you to Canada or not).

Check the box to indicate whether you are the principal applicant or the spouse, common-law partner or dependent child aged 18 years or older of the principal applicant.

It is to show details about:1.

the applicant served in any military, militia, or civil defence unit or service in a security organization or police force (including non obligatory national service, reserve or volunteer units).

2.the applicant have had membership or association with any political parties, groups or organizations which have engaged in or advocated violence as a means to achieving a political or religious objective, or which has been associated with criminal activity at any time.

3.The applicant have held any government positions in the past such as:

Civil servant
Judge
Police officer
Mayor
Member of parliament, or
Hospital administrator.
