Super Visa refused - After medical

Hi Experts,
I applied a super visa for my father and mother. My mother got the visa but my father got more medical requests. Now aftrer clearing the medical we got the letter that mentioned

"
Grounds for refusal:

• You are found inadmissible to Canada under paragraph 41(a) of the IRPA: through an act or omission which contravenes, directly or indirectly, a provision of this Act."

Please advise what could be the reason. I remember when we submitted paper application at VFS my father passport ( first page, the one which has photo " ) was a bit torn

Could it be the reason ? Please advise. Thank you

No, My father never been to outside home country . It was very first time we applied for any kind of visa

Thank you for reply.
Also, Just to update, my father did CT SCAN and Sputum test, doctor inform my father that result will come on 21st March and then there will be one more sputum to finalize the stuff.

I am wondering still they sent refusal letter in between .

Seems like it was unrelated to his health screening but it is impossible to say.

I called Medical office, they mentioned medical reports are still awaited. Not sure what could be the reason.
