Super-Visa extension

My mother got 10 yrs visa and 2 years stay stamp on port of entry. I am preparing for applying extension of stay.

1. - how many days in advance I can file application (for super visa) for another 2 yrs. stay.

2. - my mother is widow and I am only son, so it's difficult to stay alone for her; does she require to leave country at least for one day before she get another 2 yrs. term

3. - We had must have medical insurance for her and will have again until she has valid stay permission. I can submit good income proof too. How difficult is to get permission of extension.

4. - Does she need to go all medical test again, which were done at first time.

5. - I am bit confuse with 10 year visa but 2 yr. permission to stay. Can someone help me to understand benefit of 10 year visa on passport from applicant's point of view. As an applicant I need to submit application again, how would big term visa is helpful....

You are right, at back of my mind - I was evaluating sponsorship qualification but at the moment had question about super-visa extension,
