Submitted wrong form

For our dual intent LMIA we needed a job offer with an end date in it. By mistake I uploaded this one as offer of employment and not the one which says permanent job offer. Is there a way I can change the form or send it in? Otherwise we will be declined for sure. We got our AOR the beginning of August, medical passed and rest is in review. Thanks for your help.

Send in the other offer letter along with a note of explanation regretting your earlier mistake.
