I had applied for May22 intake under SDS category and received a refusal recently. The consultancy I applied through was Canam and my question is related to certain forms they had asked me to fill during the visa filing process.
My concern is about the form called as 'Student Questionnaire' which first asks us to mention the details of the consultant, then the academic and professional details since the age of 18 (explain gaps as well) and finally asks for financial information related to income of family members and work details.
Having contacted some students who have filed their visas recently, they said they have never come across this form however students who applied through Canam have claimed to filled this form.
My question to the forum is, is it mandatory to fill this form? I was handed over this form separately in a word format however this form is also a part of IMM 5818E.
Please do let me know if you have filled / submitted this form during visa filing under SDS and if it is mandatory.
