Stayed for more than 2 years - Approved Visitor Record but require a medical exam?

I have stayed in Canada for more than 2 years now to study for my master. Now I graduated and waiting for my PGWP.
My study permit expired before I could gather all the docs required for the post-grad work permit. So I applied for a Visitor Record to have legal status in Canada wait waiting.

Today (2021-11-25), my Visitor Record was approved, and I thought it was ok. However, on the evening of the same day, I received a message request for Medical Exam. I was confused because:
  1. I stayed in Canada for more than 2 years and didn't travel outside of Canada
Is this a mistake from IRCC? What can I do?

You need to submit a medical. Your extension will be contingent on you supplying one. If you don’t do the medical, your extension won’t be valid.

You still need to do the medical. If you don't, expect an email from IRCC in a few months where this will start becoming a problem. Get the medical done to avoid problems.
