Spouse paper file - eimm5669 or imm5669

Hello guys,
I am preparing for my spouse paper PR file - I have two confusion and I didn't get an answer yet from anywhere.

1. Since I am Canadian PR, a spouse will have to do a paper file and it is asking for eimm5669 (other categories ask for imm5669) so which form we should fill as some people asked to fill IMM5669 after submitting the eimm5669?
2. how many people should fill - both or just a principal applicant? please let me know - it would be a big help. Thanks,

1. You fill out the form that is linked in the guide.

2. The sponsor doesn't fill out applicant forms.
