Spousal Sponsorship - Proof of Medical?

Hi all,

Really confused here. I had my up-front medical just over a month ago for Spousal Sponsorship. I am looking over the documents to apply and have to provide proof of this medical. I have an information sheet from EMedical stamped by the Doctor who did the Medical. Does anyone know what I need to provide? I'm completely baffled by the lack of guide and information on this.

The information sheet you received form your doctor is what you should include.
