Hi,

Where can I get this Option C form as described in Sponsorship Evaluation form IMM5481 ( which is regarding personal net income ). I tried calling the toll free number and had a dead end. Can someone please help me out with this form.

Who did you call? Did you call CIC? You need to call Canada Revenue Agency.

Thank you and yes I did call CIC...lol So do I call CRA and ask them the Option C form or do I have to set up my account first and go from there ?

No you don't need an account. As long as you have filed a tax return you can request an Option C, but they will only address it to the address that they already have on file for you (ie your last tax return). You would only need a CRA login account to be able to change your address online, and other things not relevant to your question.
