Sponsorship spouse fee

So I am getting ready to pay the spouse sponsorship fee and there is 4 empty boxes one for the principal applicant other for the sponsored person, Family member of the principal applicant who is under 19 years of age, unmarried and not in a common-law relationship OR is 19 years of age or older who has been unable to be financially self-supporting since before the age of 19 due to a physical or mental condition and the right of permanent residence, confused do I select them all?

the number of applicants will be 1 principal, 1 spouse and I am confused about the other two I just stumbled upon something else that confused me spouse for canada class and family class, what's the difference between the two?

Sponsorship fee (75$)
Principal Applicant fee (475$ I think, don't remember exactly)
Right of permanent residence fee (490$)
