Sponsorship Evaluation and Option C print out for ODSP recipient?

Hi, does someone on ODSP still need to submit the Option C print out?

If so, how does one go about acquiring this?

And on the Sponsorship Evaluation, where it asks for Net Personal Income - what would an ODSP recipient put here?

Is disability considered income?(it is not taxable, after all)

Please, no guessing - hopefully someone with experience can answer these questions.
