I just want to finish all the paper and I am sorry I have more questions about the form IMM5481 Sponsor Form for Common Law Sponsorship!

what does he has to fill out on page 2 ...nothing in the guide?
anything from me the Common Law partner?

and also the question
Sponsorship Evaluation – (IMM 5481) on page 1:
Number of family members and persons included in undertakings in effect and not yet in effect
4. yourself - Answer is 1 (the sponsor)?
5.I am not sure if A or B and what answer?
6.answer A? me the common law partner? ... and where do I have to write the details they ask for?
