Hi experts, please answer some of my questions

Form IMM 1344 (12-2012) E
Question 7 ( if you are a permanent resident or a naturalized Canadian citizens, provide the following info )
Date on which you obtained this status : I got my citizenship card in 1995 , and it shows the year only. Do I have to put the exact date? if so.. where do I go to find out ( I am living in Toronto )

on the last page where it says Signatures .. if I dont have a co-signer then I just sign the top line, and my spouse signs the bottom, and omit the second line right? ( Signature of sponsor's spouse or common-law ( if co-signer)

From page 2 to page 7 .. on top of the page where it says Applicant Name and Date of Birth .. is this the info of the Sponsor or the Principal Applicant?

This Form is to be filled out by the Sponsor. You are right. On page 7, you as sponsor sign the top signature, your spouse (applicant) signs the bottom one. The middle one is to be left blank.

When you fill out the Form, the computer should automatically fill out your First Name's initial and Last Name and your birth date on top of page 2 to 7.

If you don't have the accurate date on the citizenship card, try to find the date when you received it or try to remember when your oath took place and give that date (RE: item 7 on first page)

this form was printed by someone, and that spot is blank.. should I reprint the form or I just write my Name in there with my own hand writing?

That Form should be filled out by you, if you are the Sponsor. As far as I know, when you fill out first page, the 2-7 pages should be automatically filled. Don't think, you should hand write them. Did you try filling out the Form yourself in the computer? When you download the form, it is in PDF format and you cab type in the information requested and then print it out.

If you had a previous dealing with CIC, then they may have provided you with a Client ID or UCI (in their communication with you). Don't worry, if you don't have it. You can leave it blank.
