Sponsor's Application- Sponsorship Evaluation Form 5481 vs Financial IMM1283

For the spousal sponsorship do I need to fill Sponsorship Evaluation form IMM5481 or Financial Evaluation IMM 1283?
I am now confured. Please help. Thanks.

if it is spousal with no need to meet LICO then is is IMM5481
